State Recertification Packet: Download this if you reside in: High Street Vets, Trustman Apartments, and Egmont Vets recertification forms- (The last two pages of the download comprise the the questionnaire and require legal-size paper.)
Federal Recertification Packet: Download this if you reside in: Walnut St./22 High St. Apartments, Park St., Pleasant St., Longwood Ave., Col. Floyd and 190 Harvard recertification forms - (The last two pages of the download comprise the questionnaire and require legal-size paper.)

BHA may assign charges for damages to apartments and buildings, key replacements, and other costs. BHA will NOT charge for normal wear and tear.
